Below is a listing of the services we provide through Medical Assistance Waivers as well as a brief description. The provision of each of these services may look very different depending on each individual’s goals, interests, and abilities. Behavior Support
Behavior Support Services seek to reduce challenging or maladaptive behaviors and increase positive or adaptive behaviors. Behavior Specialists meet with the individual and family, staff, or other team members initially to complete an assessment, and then will develop strategies and techniques to prioritize the individual’s needs and systematically foster change or growth. When other services are involved, Behavior Supports can act as a clinical lead for a service team and provide training and assistance to involved team members as well. This service is available for individuals on the following Medical Assistance Waivers: Consolidated, Community Living, P/FDS, and Adult Autism.
Systematic Skill Building
Systematic Skill Building Services seek to increase skills across various domains, including housekeeping, cooking/meal planning, safety, relationship building and socialization, among others. Skillsets are assessed, analyzed, and broken down into detailed tasks which can be taught in small, foundational steps. Skill Building develops the groundwork in simple skills which allows complex skills or tasks to be built. When other services are involved, Skill Building can work directly with Behavior Support to ensure consistency in strategy development, and can also provide training and assistance to other team members. This service is only available through the Adult Autism Waiver.
In Home and Community Support
In Home and Community Support Services seek to assist an individual to achieve goals as set out in an individual’s Service Plan, and vary widely based on an individual’s wants and needs, likes and dislikes, as well as their current abilities and strengths. Services can be provided both in the home and out in the community, and become highly individualized because of this. When other services are involved, Community Support works closely with Behavior Support and/or Skill Building as the primary force to assist in implementing developed strategies in both of these other categories and provide data to the Behavior Specialist and the Skill Builder. This service is available through the following waivers: Consolidated, Community Living, and P/FDS. This service is available though the Adult Autism Waiver, but is known simply as Community Support.
Nutrition Consultation
Nutrition Consultation is our newest service provision through the Adult Autism Waiver. Through this service, a Licensed Nutritionist will meet with the individual and their team, assess their nutritional needs, and provide ongoing direction and encouragement. This service also can take place in the community or in the home, which opens up the possibility of truly individualizing this service for each person.
